Programmer note: If this simple HTML page works but the ASP page does not, in IIS 2003, under Web Service Extensions top level folder, make sure that 'Active Server Pages' are 'allowed'. For a new server the default is off and you will get page cannot be found. In IIS 2008 it should show the ASP button.
